Provider production readiness

Read-only. A provider adapter existing in code never means it is live — production activation always requires an explicit env-var flag plus a real, configured adapter. There is no activation toggle on this page; flipping a flag requires an environment change and a restart. See the production provider activation and kill-switch runbook.
